Welcome to FY2010! Please take time to review quidelines for 2010 as some items have changed. There are new forms (page 2, Income Verification) that are required before we will be able to assist with services.
W-9’s
We are required, by the Internal Revenue Service to have a W-9 on file for
all tribal members who use our Social Services, this includes minors. Please complete and return to our office.
I must have this on file before I can process any applications.
